Thank you for taking part in this Teenreads.com Reader Survey. We are looking forward to hearing what you have so say so we can better program the features and reviews on Teenreads.com for you.

After completing the survey, you have the option of being entered into a drawing to receive a prize. Grand Prize will be a $100 Amazon gift certificate and there will be two other $25 Amazon gift certificates awarded as well. (Note: to be eligible for the prize you must be 13 years of age or older.)

Consider this your chance to voice your opinion to book publishers, as we will be sharing these results with them. The answers and any information you give cannot be linked to you in anyway - we guarantee your privacy.

Please note that there are three sections to this survey. As you finish each one, you will be prompted to go to the next.

To select an answer, point your cursor (the little arrow) over the circle or square next to the answer(s) that best fits, and click - a little dot or check mark/tick will appear. All the questions in each section must be answered before can you finish that section and move on to the next.

You must answer EVERY question in each section. With this in mind we have developed a possible response for EVERY question.

The entire survey should take no more than 10 minutes.

The survey is open from Noon on Monday, May 5, 2003h thru 11:59PM on Monday, May 26, 2003.

OFFICIAL RULES FOR TEENREADS.COM SURVEY DRAWING

NO PURCHASE NECESSARY. VOID WHERE PROHIBITED.

ELIGIBILITY: The drawing/contest is open to all legal residents of the United States (excluding Puerto Rico) and Canada (excluding Quebec). The following persons and their dependents are not eligible to enter or win: employees of Teenreads.com or The Book Report, Inc. and their affiliates, subsidiaries, advertising and promotion agencies, and the immediate families (spouse, parents, siblings, and children) of each of the above.

HOW TO ENTER: The Teenreads.com Survey/drawing is open from Noon on Monday May 5, 2003 thru 11:59PM on Monday May 26, 2003. While the survey is posted on Teenreads.com, teens have the option of entering the drawing after completing all three sections of the survey found on http://www.teenreads.com/community/0305_reader_survey.asp

NOTE: To be eligible for the prize you must be 13 years of age or older.

HOW TO WIN: Total number of eligible entries received determines odds of winning. From all eligible entries received, Teenreads.com will randomly select three (3) winners on or about Monday June 2, 2003. The Grand Prize winner will receive one (1) $100.00 Amazon gift certificate. The other two winners will each receive one (1) $25.00 Amazon gift certificate. In the event the Potential Winner is a Canadian resident, he/she (or, if winner is a minor, his/her parent or legal guardian) will be required to correctly answer a time-limited skill testing mathematical question to be administered by mail to receive the prize.

PRIZES: Teenreads.com will notify the selected winners on or about Monday June 2, 2003. The winners will have 30 days after notification to respond for collection of the prize. The Grand Prize winner will receive one (1) $100.00 Amazon gift certificate. The other two winners will each receive one (1) $25.00 Amazon gift certificate.

LIMITATIONS OF LIABILITY/RESERVED RIGHTS: Neither the Sponsor, nor the parent, subsidiary or affiliate companies, nor the promotional agencies shall have any obligation or responsibility with regard to (i) entries that contain inaccurate information or do not comply with these rules, (ii) entries, prize claims or notifications that are lost, late, incomplete, illegible, unintelligible, damaged or otherwise not received by the intended recipient, in whole or in part, due to computer or technical error of any kind, (iii) telephone, electronic, hardware, software, network, Internet or computer malfunctions, failures or difficulties, or (iv) any damages or losses of any kind caused by any prize or resulting from acceptance, possession or use of any prize. The Sponsor, in its sole discretion, reserves the right to disqualify any person tampering with the entry process or the operation of the Web Site or otherwise violating these rules. The Sponsor further reserves the right to cancel, terminate or modify the Sweepstakes if the Sweepstakes cannot be completed as planned because of infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort.
